Instructions

To apply, the first thing to do is contact Dr. Cartwright to enroll in the auditions. She'll add your ACES email address to the audition roster so that you have full access to the application and all resources in Canvas.

All audition forms must be received by the deadline to be considered! You will need to complete a prescreen video prior to submitting this form!

The essay question is a file upload, so you'll need to write it as a Word doc or other accepted format first and upload it when prompted. The question asks you to tell us about your experiences with jazz, why you are drawn to it, and why you are applying for the A-JAM mentorship program. Include the following information: how you heard of the program, what you know about it, what you hope to gain from the experience, and what you feel you can contribute to the program. Don't forget to do a spell and grammar check before you save your file.

Be sure to fill out each question completely. Incomplete questionnaires will be discarded. You may edit and re-submit the audition form as many times as you like before the deadline; only your most recent submission will be kept.

If you have any questions, feel free to contact Dr. Cartwright via the Canvas inbox. For technical issues related to Canvas, contact Canvas support via the question-mark icon to the left of the navigation bar.

Only registered, enrolled users can take graded quizzes
